thanks for your question.

"nipote" -> singular for both masculine and feminine nouns (grandson, granddaughter, nephew, niece)

"nipoti" -> plural

You can distinguish between masculine and feminine thanks to other words in the sentence. 99% of the time, "nipote" will be introduced by an article ("il" for the masculine, "la" for the feminine) or by a possessive ("mio nipote" VS "mia nipote"). As for grandson VS nephew, granddaughter VS niece, if that's not clear from the context, you can just ask or specify. For example, if someone is too young to have grandchildren, "nipote" will most likely refer to their sibling's children. If you're the one talking, you could say something like "mia nipote, la figlia di mio fratello" (my niece, my brother's daughter).

Julie, for one thing, it can be either masculine or feminine (mia nipote vs mio nipote) which helps.

Beyond that, in my experience there's a theoretical confusion but it's not usually a problem. When my middle-aged Italian friend points to a picture of a baby and speaks about her "nipote," we assume it's her grandson. If he were actually her nephew, she could easily clear it up with "e il figlio de mia figlia" and it's all good from then on. Different languages tend to deal with family relationships differently. In Hawaiian, kuaสปana is an older sibling of same sex while kaina is a younger sibling of same sex, but in English we get by just fine without that distinction.
